Continuity in Care

Caring Homes approach to care delivery focuses on delivering continuity in care.

What is Continuity in Care and Why is it Important?

Continuity in care is care that is delivered in a continuous way to ensure a consistency in the service a person is receiving.

At Caring Homes we have created a number of different initiatives which are centred on delivery continuity in care.

Summer

Key Workers

If you decide to choose a Caring Home, you will be allocated a Key Worker, who will be your main contact in terms of personal care.

Your Key Worker will be a Healthcare Assistant, assigned to oversee your day to day care, ensuring that any other member of staff who supports you is fully aware of your individual preferences, choices, likes, dislikes etc.

Your Key Worker will be your main carer, and will support you with letter writing, booking appointments, booking taxis, dry cleaning etc

Names Nurses

If the Caring Home that you are resident in is a Care Home with Nursing, you will be allocated a Named Nurse, who will be the main contact in terms of nursing support.

Aromatherpay

Key Workers & Named Nurses

The Key Worker and Named Nurse Structure, ensures continuity in care through their gaining an in-depth understanding of your needs, choices, likes, dislikes etc.

Through this structure you will be able to develop positive relationships, companionship and consistency in all areas of care and support that you need.

Home Managers

Caring Homes philosophy and mission is to improve the quality of life of the people who we are privileged to care for.

With our belief that first and foremost, we are a care provider, each of our Home Managers, is a qualified nurse, who has been hand picked for their experience and expertise in providing quality care and support to elderly people.

Our search and selection process for Home Managers is rigorous and robust. Whilst a Nurse qualification is essential, there are other attributes which are high on our agenda when recruiting a Home Manager.

Our expectation is that all Home Managers, will have considerable experience in delivering first class professional care to elderly people, will have management experience, will be able to evidence a shared philosophy of quality care delivery, will have good standards in inspection reports from previous roles and will share our cultural beliefs.

Peripatetic Managers

In furtherance of our commitment to deliver consistently high quality services, we have developed a peripatetic management structure to support our regional operations teams during absence due to sickness, annual leave or to facilitate cover during training.

The peripatetic management team is a team of highly qualified and motivated individuals, all qualified nurses, and all sharing the Caring Homes philosophy.

This team offers valuable support, in the Caring Homes way to ensure that a home continues to operate under our ethos, even when the manager is unable to be at work. This valuable resource helps us to minimise the need for “Agency Cover” and further supports our commitment of ensuring consistently high standards in all that we do.
